More Event Information

You are cordially invited to join us for the 9th Annual ForMatt Foundation Golf Tournament to be held on Friday, August 11th at the Derryfield Country Club in Manchester, NH.  The ForMatt Foundation is a non-profit organization formed to honor the joyful spirit and generous heart of Matthew Choiniere who passed away in November of 2008 at the age of 22. The ForMatt Foundation provides grants to children making it possible for them to participate in healthy activities that would not normally be within their means. 

 THERE ARE A NUMBER OF WAYS YOU CAN GET INVOLVED:  

• Plan to join us for a great day of golf & invite your friends and family to play in the tournament, too! The fee is $100.00 per player ~ Registration deadline Friday, August 4, 2017. Includes Green fee, cart, and lunch.


• Sponsor a hole for a $100.00 donation per sign. Your business name or special message will be printed on a sign and placed at one of the 18 holes..  


• Make a donation in the amount you wish to give. 


• Donate a great prize for our raffle on the day of the tournament.


• Help us to spread the word and invite your friends!

EVENTBRITE DIRECTIONS:

You may make a Donation in any amount OR select “Enter donation” to pay $100.00 per Player and/or Hole Sponsor Sign. You may register as a single player or pay for your teammates.

After you select “Register”, you will be prompted to enter additional registration information. Under the “Other Information” section, please be sure to enter the names (and email addresses) of each player in your group that you are playing with. If applicable, please also indicate here which players you are paying for. Individual registrants will be assigned to a foursome unless foursome is specified in this section. Sponsors, the “Other Information” section is where you will add your business name and/or special message to be printed on your sign.

TO AVOID SERVICE FEES: go to http://formattfoundation.org/Matt/Golf.html to print the registration form to complete and mail with a check.

All proceeds will support our mission. Through participation in fun and engaging activities children develop important social skills, learn about healthy choices, and experience positive childhood memories that will last a lifetime.  Research shows that when children have positive experiences at a young age, they are better able to manage adversity later in life. Our goal is to continue supporting children for many years to come.
