Event Details:

• Registration begins at 9:00 am
• Shotgun at 10:30 am
• Light breakfast and box lunch during outing
• Dinner and awards ceremony after outing

Fees: $600 per foursome; $150 per individual golfer and includes:

• 18 holes of golf
• Cart rental
• Unlimited beverages
• Breakfast, lunch, and dinner plus snacks
• Goodie Bags
• Entry to contests such as hole-in-one, longest drive, closest to the pin, longest putt

Who we are

Shalom Health Care Center (Shalom) is a 501c3 non-profit primary care organization that aims to provide quality healthcare services that are patient-centered, affordable, and accessible for all in our community.

We provide our patients with low-cost primary and behavioral health services covering a range of chronic conditions, as well as substance use disorder treatment. We offer our uninsured patients a sliding fee discount program, that determines their nominal fee based on their household size and income level. Our vision is health equity for all in our community, and we work daily to provide services that our community historically has not had access to due to socioeconomic inhibitors.
